Our Physio team can support people reach their goals whilst living with the challenges of disabilities and chronic conditions such as:

HOW CAN WE HELP?

  • Your NDIS support team (plan managers, support coordinators)

We provide therapy, advice and advocate for people in their daily life and help them participate in the community and reach their goals.

 

We can help in your preferred environment this includes in our clinic, in your home or at work.

LISTEN

Our team will listen to you to understand your unique challenges and NDIS goals,

this will include:

  • learning about you and your health history
  • assessing your ability to move and function
  • Collaborating with other NDIS providers
  • Recognising challenges that exist for you
  • Providing a thorough assessment

PLAN

We plan our Physiotherapy services to support your desired lifestyle both short and long term.  Our physios use evidence based practice to develop a shared plan considering your ‘bigger picture’. This plan includes an Individual Service Agreement tailored to support your goals.

DO

Our team do Capacity Building to Improve Daily living this means:

 

  • Hands on therapy to keep you moving and pain free
  • Education and advice about managing your disability or condition
  • Exercise to maintain/improve your strength coordination and posture
  • Training your support people to assist with your goals (mobility, transfer, exercise programs, simple therapy)
  • Developing fundamental movement skills (gait, transfer practice)
  • Balance training
  • Development of resources for you and your support team
  • Assistance to enter or maintain employment through assessment of work spaces
  • Promote and support physical activity
  • Recommendation of other Allied Health Professionals as required
  • Identify opportunities for reaching further (dreaming bigger)

REVIEW

We continue to review, together, our shared program to ensure that we are on track to meet both your short and long-term goals. This allows us to adapt and change as your needs.

COMMUNICATE

To make sure we are working towards your goals we communicate clearly and regularly with you and your NDIS team.  We also talk with other wider community supports at your request such as your boss, your school, other health professionals.  We advocate for you to increase your independence, inclusion, and social and economic participation in life.
